A) Documents Required
1) An Application for Passport

2) Two (2) Color Photos :
- The photos on white background should be less than 6 months old.
The size of the face is approximately 2.5 x 3.5 cm and should be the front view.(Size: 2" x 2")
- The hats, head-gears, or sunglasses are not allowed.
3) The old passport (original) and a copy of the passport (the pages with personal information)
4) Fees and other Documents Required (For each category)
* Male Applicants' Ages between 25 and 35: please refer the following item B)
* Fees should be paid in cash or money order

B) Following Individuals have to submit Additional Documents
1) Male Applicants Ages between 25 and 35:
- Proof of the completion or extension?from military services
(a copy of the page of the passport on which the embarkation records is stamped or the certificate of completion)

2) Family Census Register (Ho Juk Dung Bon)
IF a) you were under the age of 14 when you departed Korea and are new 14 years of age or older,
or b) your current passport does not have your Korean social security number.
- Military record, IF you are a male and between age of 25-35
3) For the Addition or Deletion of Married Name:
A Certificate of Marriage or Divorce or A copy of Family Census Register (Ho Juk Dung Bon) |
C) How to apply by mail
1) Use USPS Express Mail. Provide a self-addressed stamped envelope with the forms and documents.
Please include $16.25 stamp for express service. You may use DHL, Airborne, UPS or FedEx ONLY IF you have an account with them. Credit card charges are NOT acceptable.
2) This office is not responsible for documents misdirected in the mail. |

| II. Extension of Validity |
| Request for extension of validity can be submitted 1 year before or after the expiration date of the current passport. However, only a one time extension is allowed due to the laws that state that the validity of a passport cannot exceed 10 years from the
issue date. |

- Hats, head-gear, and sunglasses are not allowed.
3) Both the original and a photocopy of the passport (the pages with personal information)
4) Fees and other required documents (specific to each category)
-Fees must be paid by Money Order if requested via postal mail please
make payable to the Korean Consulate General(Cash accepted in person). |
